Ihre Aufgaben
Aufnahme und Pflege von Patientendaten
selbständige Planung, Organisation und Durchführung von Diabetesberatungen aller Formen des Diabetes mellitus
Durchführung strukturierter Therapie- und Schulungsprogramme / Einzelschulungen
Mitarbeit bei wissenschaftlichen Studien
Durchführung von administrativen Tätigkeiten (Dokumentation)
interdisziplinäre Zusammenarbeit mit Ärzten
Ihr Profil
Sie verfügen über eine abgeschlossene Ausbildung im Bereich Diabetesberatung DDG
erste Berufserfahrung im beschriebenen Aufgabengebiet
Sie sind berechtigt zur Durchführung der bei der KV Sachsen abrechenbaren Schulungsprogramme (KV/ZI Programme, Medias etc.)
Kenntnisse in der Insulinpumpentherapie und der Glukosesensortechniken
Sie können die Anforderungen nach DMP umsetzen/anwenden
Sie bringen ein hohes Maß an Zuverlässigkeit und Belastbarkeit mit
im Umgang mit Patienten agieren Sie stets freundlich und korrekt
Kenntnisse im Bereich Abrechnung (ambulant/ stationär) sowie gute EDV-Kenntnisse sind wünschenswert
